  • Configure, build, and maintain the Epic Health Planet module to support population health initiatives.

  • Collaborate with clinical and operational leaders to design workflows that support care management, preventive care, and chronic disease monitoring.

  • Analyze population health data and metrics to support the design and implementation of targeted care strategies
