Qualifications
~ Education: Masters Degree in Social Work (MSW)
~ Licensure: Active NY State LMSW License
~ Experience: Minimum of 1 year of experience
~ Technical Skills: Proficiency with Electronic Health Records (EHR)
~ Soft Skills: Empathy, cultural competence, and a patient-centered approach
Key Responsibilities
~ Conduct comprehensive psychosocial assessments and diagnoses for patients and their families
~ Provide evidence-based individual, group, and family therapy
~ Develop, implement, and monitor individualized treatment plans in collaboration with a multidisciplinary healthcare team
~ Perform crisis intervention and risk assessments, implementing appropriate safety plans as needed
~ Advocate for patients and families to ensure access to community resources, social services, and financial assistance programs
~ Provide care coordination, including patient education, discharge planning, and referrals to appropriate follow-up care
~ Maintain timely, accurate, and confidential patient records in compliance with all state, federal, and organizational policies
~ Participate in case conferences, team meetings, and ongoing professional development opportunities
