In this role, you will conduct psychosocial assessments, provide brief solution-focused counseling, and develop collaborative treatment goals. Your efforts will help patients overcome barriers, access community resources, and ensure continuity of care across inpatient and outpatient settings. **What You'll Do:** + Conduct comprehensive psychosocial assessments for patients and families. + Develop treatment plans that address emotional, social, and resource-related needs. + Provide brief, solution-focused counseling and facilitate discharge or care planning. + Advocate for patients and coordinate referrals to community-based services. + Collaborate effectively with interdisciplinary care teams. + Document all assessments, plans, interventions, and outcomes clearly and promptly. + Meet departmental productivity expectations and performance standards. **What You'll Bring:** + **Education:** Master's Degree in Social Work + **Licensure:** **Limited Licensed Master Social Worker (LLMSW)** or **Licensed Master Social Worker (LMSW)** by the Michigan Board of Social Work. + **Experience:** Minimum of one year of social work experience in a healthcare setting. **Key Competencies:** + Strong psychosocial assessment skills and clinical judgment. + Ability to navigate complex family dynamics and high-acuity situations. + Excellent crisis support and problem-solving abilities. + Outstanding interpersonal and empathetic communication skills. + Proficiency in engaging patients and care teams in effective treatment planning. + Knowledge of community resources and continuity of care principles. + Accurate, timely documentation in medical records and reporting systems. **Why Trinity Health?** As a mission-driven, people-centered health system, we are committed to providing compassionate care and promoting the well-being of every person we serve.
